Why are certain forms required by the Ontario Ministry of Health (MoH)?

The MOH occasionally requires supporting documentation to be submitted alongside the claim submission.  Various codes within the Schedule of Benefits (SoB) require specific forms to be completed by the physician and provided to the MoH for review. The Ministry utilizes these forms as a means of verification that the required criteria have been met under the Act; to which the code applies. 

Although completion of forms is mandatory for certain codes, not all forms need to be submitted to the MoH for review. Rather, they must remain in the patient’s medical history as part of their record. These forms would be required upon MOH request, therefore, should be readily available. 

What forms are required to be completed for Claim Submission?

  • Consultations and Assessment under the Mental Health Act (K620, K623, K624, K629) 
  • Community Treatment Order (CTO) (K887, K888, K889) 

What forms are required to be completed but do not need to be submitted as part of Claim Payment?

  • Ontario Hepatitis C Assistance Program (OHCAP) (K026, K027) 
  • Home Care Application (K070) 
  • Mandatory Blood Test Act (K031) 
  • Completion of Northern Health Travel Grant Application (K036) 
  • Completion of Long-Term Care Health Report (K038) 
  • Certificate of Death (A771, C771, W771) 
  • Medical Condition Report (K035) 

Additional Forms not covered under the Health Insurance Act but paid by the MoH, acting on behalf of the Ministry of Community and Social Services (MCSS): Ministry of the Attorney General, Ministry of the Solicitor General and the Workplace Safety and Insurance Board (WSIB)

  • Appendix F of the SoB

Uninsured and Third-Party forms

At times, patients may approach a physician to complete a necessary form for various life situations.  These forms may be billable to the patient or a third party.  Note: billing a patient is at the physician’s discretion and the Forms Table is only a guide.  

There are also various forms that are required by legislation to be completed but are classified as an “uninsured” service and cannot be submitted to OHIP for payment. They are not to be charged to the patient either (Regulation 552)
